Welcome Family Choice Medical Group providers to the connect platform

To log into connect please navigate to https://connect.familychoiceportal.com

Registration

If you DO NOT have a CONNECT Provider Portal account, click here to REGISTER. Please use your Registration Code in order to expedite access to eligibility, referral and claims information. The registration code was sent to your mailing address and/or fax – if you cannot locate, please contact our Technical Support.

  • Upon registration you will be asked to create your login credentials and validate your email address. Once your email is validated you will receive a confirmation email shortly after and we will start to configure your account.
    TIP: To save you time, we recommend that provider practice managers/administrators register first – and starting next week they will be able to add additional users without the need for their staff to go through the registration process. However, everyone can register at this time if they want to.
  • The portal will not yet be available for your use until 3/1.

On Monday, March 3rd at 8:00 AM you will have full access to the CONNECT Provider Portal.

  • All current and historical eligibility, provider, auths and claims data will be available on the portal.
  • You will be able to submit all referrals.

Training

Portal training will be available on Thursdays from 12:00pm-1:00pm. During the scheduled time click the meeting link below to join a training session via Microsoft Teams:

  • February 13th, 12pm-1pm
  • February 18th, 12pm-1pm
  • February 20th, 12pm-1pm
  • February 25th, 12pm-1pm
  • February 27th, 12pm-1pm
No prior registration is required for training sessions.
